Per Minor Students
I, the parent or legal guardian of the attending child, authorizes participation of my child in classes and other ministry related events while participating in Tribe Nawaar LLC/Jennifer Secrist Goran’s classes (hereafter referred to Tribe Nawaar) This includes all activities and transportation to and from all activities.
Per All Students
I understand that my/my child’s participation is voluntary and that dance-related activities are conducted by Tribe Nawaar staff, volunteers, and guest faculty. I understand that Tribe Nawaar is solely responsible for all aspects of ministry related activities including the selection and supervision of all people conducting activities. I understand that dance is an area of study that by nature involves risk of injury and even death from various causes, including but not limited to accidents, falls, strenuous and prolonged physical activity, dehydration, illness, collision or dispute with other participants, weather-related injuries, training and performance area equipment defects and negligence of supervisory persons. I further understand that the art of dance and nature of dance instruction may sometimes require a teacher to have physical contact with me/my child. I understand that all physical contact/touch will happen in a caring and appropriate manner. I understand that all emotional, physical and general or specific health concerns should be cleared with my doctor before participating in any Tribe Nawaar activity.
Online Addendum
While participating in online classes I understand it is my responsibility to create a safe space to for me/my child to dance in. I understand movements and dance steps taught online will need to be modified by me/my child to accommodate the area I have chosen to dance in.
Photos & Videos
I give Tribe Nawaar my consent to freely use photos and/or videos of myself/my child for advertising on the website, newspapers, and any promotional materials. I understand that it is my responsibility to let the Administration know if I do not want our names used in conjunction with our photos.
In consideration of the privilege of my/my child’s participation in Tribe Nawaar activities, and on behalf of me/my child and me as parent/guardian, I hereby release, hold harmless and indemnify, and covenant not to sue Tribe Nawaar, and all Tribe Nawaar employees, volunteers, insurers, and other people associated with Tribe Nawaar (including but not limited to sponsors, parents, officials, drivers, and organizations) as to any and all claims of me/my child and other family members for personal injuries suffered by me/my child, property damage, medical expenses, and economic loss arising directly or indirectly out of my/my child’s participation in Tribe Nawaar activities. This also includes any first aid, medical care or treatment provide to me/my child in the event of injury or illness while participating in Tribe Nawaar activities; I further release any claims that may not be released under applicable law. This Release of Liability shall be as broadly construed as allowed by law to include all claims and rights that the participating student, that I as parent/guardian, and that other family members may have. If any provision of the Release of Liability is deemed invalid, the remaining provisions shall remain in full force and effect. This Release of Liability shall be binding on me, my family, heirs, next of kin, legal representatives, beneficiaries, successors and assigns.
My participation in any Tribe Nawaar activity indicates that all information provided during registration is true and accurate, and that I fully understand and agree to all statements herein.
